Dental Plan

The AWANE Dental Plan is designed to provide employees with maximum benefits, very little fine print and complete freedom to use any dentist. The Dental Plan is considered an employee reimbursement plan, whereby, all checks are paid directly to the employee, not the dentist. There are no claim forms to complete, an employee only need submit an itemized bill from a dentist. Checks for reimbursement are usually paid within a week.

HIGHLIGHTS OF THE DENTAL PLAN INCLUDE:

Generous total benefit of $1,300 for 2006; $1,500 for 2007 (per calendar year, per individual):

• Coverage for diagnostic and preventive services, paid at 100%, no deductible.
  (Examples include cleanings and X-rays.)
  Note: Oral exam & cleaning benefit limited to $75 for 2006; $90 for 2007.
• Coverage for basic restorative services, paid at 80%, no deductible.
  (Examples include fillings and root canals.)
• Coverage for major restorative services, paid at 70%, no deductible.
  (Examples include bridges and crowns.)

Orthodontic benefit of $1,000 per dependent (age 19 or younger), if elected:

• Coverage for orthodontic services, paid at 60%, no deductible.
