These are some of the more frequently asked employee questions.

How do I use the FIS/Metavante Debit Card?

The FIS/Metavante dental debit card is not worth anything until AWANE funds it. You go to the dentist, have the work done, and ask them to fax the bill to 1-877-218-8216. It takes AWANE 3-4 minutes to process the bill, fund the card at FIS/Metavante and call the dental office back with the approved amount. They then swipe the card for the approved amount.

What if I lose the card?

Call your customer service rep at AWANE, and we can cancel the current FIS/Metavante number and issue a new card.

What is my yearly maximum?

For 2010 the yearly maximum is $1,500 per person for the AWANE Dental Plan and $1,000 per person for the AWANE Dental Industry Plan.

Do I need to go to a specific dentist?

We do not have a specific network. Go to the dentist of your choice.

Do I need to get pre-treatment estimates?

No - we do not process claims based on a pre-determined schedule. We process on a percentage basis, based on the category of the claim up to the yearly maximum.
