These are some of the more frequently asked employee questions.

How do I use the MBI Debit Card?

The MBI dental debit card is not worth anything until AWANE funds it. You go to the dentist, have the work done, and ask them to fax the bill to 1-877-218-8216. It takes AWANE 3-4 minutes to process the bill, fund the card at MBI and call the dental office back with the approved amount. They then swipe the card for the approved amount.

What if I lose the card?

Call your customer service rep at AWANE, and we can cancel the current MBI number and issue a new card.

What is my yearly maximum?

For 2006 the yearly maximum is $1300 per person. For 2007 the yearly maximum is $1,500 per person.

Do I need to go to a specific dentist?

We do not have a specific network. Go to the dentist of your choice.

Do I need to get pre-treatment estimates?

No - we do not process claims based on a pre-determined schedule. We process on a percentage basis, based on the category of the claim up to the yearly maximum.
