Long-Term Disability

The AWANE Long Term Disability Program will help you and your eligible employees insure a portion of your/their income if you/they should become disabled.

As a dues-paying A.W.A.N.E. member, the owner or his/her delegate receives a Long Term Disability policy at the $1,800 per month benefit level (with a six-month waiting period) at no cost. This product is available to all full time employees (30 or more hours per week).

All your full time employees can participate in this valuable program on an employee-pay-all basis.

Long-term disability policy benefits are determined by your annual salary according to the schedule below.

PLEASE NOTE THE FOLLOWING:

• Employees must work full time (30 or more per week) for eligibility.
• Total participation must be at least 25% of eligible employees.
• Employee-pay-all premium; therefore, benefits treated as “non-taxable” income.
