These are some of the more frequently asked employee questions.

I need a new medical or RX identification card.

Contact customer service rep at AWANE to reorder a card.

Change of address?

Contact AWANE (not Anthem). We implement address changes and send them to Anthem and Express Scripts.

Can I change plan type during the year?

Plans can be changed (ie Industry to Gold) during open enrollment each year (January).

Can I add my spouse, children, etc. to the plan?

Contact customer service rep at AWANE to discuss particulars. Under certain circumstances a dependent can be added off cycle, but for the most part, additions are made at open enrollment.

How long can my dependent children stay on the plan?

Until age 19, or if they are a full time student carrying 12 or more credits they can stay on a parents plan until age 25. Full time status is verified twice a year.

Specific benefit questions?

Contact either the customer service number on your card for Anthem Blue Cross Blue Shield or contact your customer service representative at AWANE.

Specific problems with a claim?

Contact Anthem directly for the most immediate access to answers and their system. Any problems that don't seem to get resolved - direct to AWANE and update on the progress to date.

Why did I pay more than my co-pay for my prescriptions?

Prescription drugs are subject to a yearly deductible ($ amount depends on the plan). Once the deductible has been met, members are charged a co-pay for their drugs.

Monthly billing - My statement says February 2006. What month is this for?

Your monthly medical premium is always billing for the prior month. Your February 2006 premium statement is due on Feb 15, 2006, and is paying for coverage for January 2006.

My company chose a 60 day waiting period. Can a new employee come on the medical coverage at any time after that?

A new employee must come on the plan on the first of the month following the completion of the waiting period. They are only eligible to come on at that time, or at open enrollment (January).

Who notifies my ex-employee of his/her Cobra rights?

AWANE notifies all individuals who leave the plan of their Cobra rights. We handle all cobra notification, billing, enrollment, disenrollment.
